The Child Care Center offers following forms to download:
- Registration Form
- Child health form
- Electronic monitor infant room
- Medication authorization
- Transportation authorization
- Field Trip permission
Download the appropriate form(s), fill it out, sign and submit to the Child Care Center.
Forms information
The Registration Form must be completed for each child who will be enrolled in the program, and must be updated whenever information changes.
The Child Health form is to be completed by a Parent/Guardian and a Physician.
